Who we are

Sanimedica was established in 1984 as a multi-specialty practice working mainly in cardiology, geriatrics, functional rehabilitation, sports medicine and occupational medicine. Since 2005 it has been running the Bellosguardo residential nursing home in Civitavecchia, accredited by the Lazio Region.

Capacity and units

58 beds: 19 in the high-maintenance unit and 39 in the low-maintenance units. Single and double rooms in the high-maintenance unit; double, triple or quadruple rooms in the low-maintenance units. The bathroom is shared with the other residents of the room or, in some cases, with the residents of the adjoining room, up to a maximum of four people. Many bathrooms have grab rails for disabled people and shower chairs. Residents cannot choose their bed; placement depends on availability and, for ward requirements, the bed may not be permanent.

Facilities

Gym, one occupational therapy room per unit, hairdressing room, one dining room per unit, visitor rooms, mortuary room with mourning room, chapel, visitor toilets, one treatment room per unit, medical office, administrative office and outdoor areas that relatives and residents may use, including for outdoor occupational activities when organised.

Guiding principles

Equality, impartiality, continuity, freedom of choice, participation, efficiency and effectiveness, in accordance with the Directive of the President of the Council of Ministers of 27 January 1994.

How to be admitted

Admission is granted only through a request by the attending physician, approval by the CAD of the local health authority and inclusion in the waiting list. The admission date is agreed with the facility and with the local health authority by 10.30 a.m., except for residents transferred from hospital.

Documents to bring on admission

Admission authorisation issued by the CAD, SIRA form; certificate of freedom from contagious diseases issued by the general practitioner; SARS-CoV-2 swab taken within the previous 48 hours with a negative result; health card and any exemption certificate; identity document; tax code; any clinical documentation, including previous hospital stays; therapy prescription issued by the general practitioner or by the hospital; indication of the general practitioner chosen within the local health authority.

Personal belongings

Slippers, closed shoes without laces, pyjamas or nightdress, dressing gown, tracksuits, comfortable clothes, towels, personal underwear, everything needed for personal hygiene, incontinence pads. Quantities are listed in the handbook given to relatives.

Admission and Individual Care Plan

The resident is welcomed by the medical and nursing staff, by the nursing coordinator and by the rehabilitation professionals and undergoes a multidisciplinary assessment including assessment scales. The Individual Care Plan is drawn up within 48 hours of admission on the basis of the resident's needs, problems and preferences, is completed by each professional, is shared with the family and is reviewed periodically by the team in order to check whether the objectives have been achieved and to confirm or amend the plan. The assessment unit of the CAD defines the individual plan together with the general practitioner and the medical director of the facility.

Medical care

A physician is present from Monday to Saturday, except public and extended holidays. Responsibility for treatment remains with the general practitioner chosen by the resident: the physicians of the facility inform the general practitioner promptly, who then visits the patient and decides on the diagnostic and therapeutic pathway, any referral to the emergency department, instrumental tests, specialist visits and laboratory tests. Medical visits follow the clinical surveillance set out in the Individual Care Plan and are also carried out at the request of the nurses or of the resident.

Specialist services

Through the services of ASL Roma 4, and therefore free of charge for the resident, general practitioners may consult specialists in geriatrics, psychiatry, diabetology and otorhinolaryngology (physiatry). Blood samples and other biological samples are taken when needed by the internal staff and sent to the laboratory. More complex laboratory tests and diagnostic imaging are carried out at the diagnostic services of the Civitavecchia local health authority or at other affiliated facilities, with the consent of the resident or of the guarantor, who bears the related costs and transfers.

Nursing care

Nurses registered with the professional register are present 24 hours a day. They administer chronic and acute therapy prescribed by the physicians, perform dressings, manage pressure ulcers and bladder catheters according to good clinical practice, assess pain and skin condition, supervise the restraint devices prescribed by the medical staff and perform blood samples and electrocardiograms when prescribed.

Basic care

Social care services are provided by basic care assistants and cover primary needs: personal hygiene, use of the toilet, serving of meals, hydration, help with mobilisation and transfers and with all activities of daily living. Qualified social care and healthcare staff are present 24 hours a day, every day of the year.

Emergencies

The physician intervenes in emergencies, stabilising the patient where possible and assessing referral to the emergency department with a referral report. When the physician is not present, nurses may use telephone consultations, which do not replace the intervention of the 118 emergency service: once the situation has been assessed, nurses call 118 and arrange referral to the emergency department using the dedicated form. At night and on public holidays nurses may call the local out-of-hours medical service, which guarantees intervention at the facility. The family is informed at the same time and the resident is transported by an ARES 118 ambulance.

Rehabilitation and occupational therapy

Physiotherapy and rehabilitation are provided by a physiotherapist holding the relevant three-year degree, individually or in small groups, in the gym and in the units, in order to maintain residual abilities and achieve greater physical autonomy. The occupational therapist and professional educator organises occupational, expressive and cognitive stimulation activities aimed at maintaining residual skills, recovering lost abilities, developing socialisation and integration and preventing cognitive decline. The large outdoor areas allow motor and cognitive rehabilitation paths, an Alzheimer garden, gardening and horticultural therapy.

Recreational activities and involvement of relatives

Choral and motor activities, cultural stimulation, expressive activities, recreational moments, shows, birthday parties and anniversaries, workshops of manipulation, painting, decoration and sewing. Activities follow a schedule displayed in the facility. On Sundays recreational and workshop activities are held together with relatives, who are also involved in special events.

Catering

The catering service is centralised and meals are supplied by a specialised and authorised external company. The menu and special diets are defined by the dietician together with the Medical Director, are displayed in the common areas and change with the season. The HACCP self-monitoring system ensures that meals are prepared and served according to proper hygiene and health standards. Meals are served in the room to bedridden residents and in the dining room to all others, at the times set by the health management. The choice of meals in accordance with the resident's religious beliefs is guaranteed upon prior notice.

Cleaning

Rooms and common areas are cleaned daily with compliant products, by dedicated staff other than the staff providing basic care to residents.

Laundry

The service covers only the washing of the room linen supplied by the facility. Personal linen and garments of particular value or sentimental value must be treated privately; a dedicated contract with affiliated laundries, at a fixed and guaranteed price, may be signed with the administrative management. Where relatives wash personal linen at home, garments must be collected and returned three times a week. On admission the resident hands the offices a list of the quantity and description of the clothing, updated whenever new garments are added.

Additional services on payment

Hairdresser, beautician, with manicure and pedicure, and chiropodist are provided by external professionals according to an agreed price list and can be booked at the reception desk. A bar service with vending machines for hot and cold drinks and snacks is available in the basement, next to the gym.

Religious service

A priest ensures at least a weekly presence for the celebration of Mass, the administration of the Eucharist to residents who cannot attend Mass, confessions and the sacrament of the sick. Residents of other religions are helped in requesting spiritual assistance according to their own faith: once the request has been received, the health management contacts the relevant religious community.

Social work service

A social work service is available in the afternoon from Monday to Friday to support the resident in dealing with social, family and behavioural issues and with administrative procedures, including requests for aids to the local health authorities. Social worker Dr Silvia Bozzi. For information on the outcome of a procedure it is always possible to contact the Social Service on 0766 220467.

Protection of vulnerable situations

When the resident is taken into care, any condition of vulnerability, including temporary, is identified through the forms used for the admission of new residents. Once vulnerability has been identified, the social worker informs the health management of the need for any referral to the municipal social services and for social integration of the healthcare intervention already in place.

Fees and payments

The daily fee is divided between a share borne by the Regional Health Service and a share borne by the resident, with the possible contribution of the Municipality according to Lazio Region rules; the share borne by the resident is set by the Lazio Region and may be changed by the Region. The resident or the guarantor pays the monthly fee in the amount indicated in the residence contract, exclusively by bank transfer, by the 5th day of each month; extra expenses and services are paid in the same way and by the same deadline. The management issues a monthly invoice. On admission a non-interest-bearing deposit is paid, in the amount set by the administrative management, as a guarantee for the expenses incurred on behalf of the resident; it cannot be offset against monthly fees. Late payment entails interest at the legal rate increased by five percentage points.

Temporary absences

For periods of absence due to visits to relatives, holidays, external medical visits or hospital stays, the accommodation fee remains fully due, in accordance with regional rules. Visits to relatives and holidays require at least 7 days' notice; no notice is required for medical visits and hospital stays. Failure to give notice entails a penalty of 30 per cent for each day.

Discharge and planned readmission

Residents may be discharged, including temporarily, for admission to another healthcare facility, for a return to the family or for other reasons, with the right to be readmitted on the scheduled date; for the whole duration of the temporary discharge the co-payment share remains due. On leaving, a discharge letter is issued containing the diagnosis, the tests performed, the results, the treatments provided and the treatments recommended. Discharge against medical advice requires the signing of a statement, recorded in the personal file, releasing the facility from any liability. Where the resident stays outside the facility, administrative discharge is carried out and readmission requires repeating the assessment and reintegration pathway through the CAD.

Copy of the personal record

A copy of the personal record may be requested on discharge at the administrative office, open from Monday to Friday from 9.00 a.m. to 1.00 p.m., and is available within 10 days of the request. It may be collected in person, by a relative or by another person holding a full proxy together with a photocopy of the identity document of the resident. Residents living outside Civitavecchia may request delivery by priority mail, upon written request and with postal charges applied.

Prevention of healthcare associated infections

Hand hygiene is performed with alcohol-based hand rub or by washing when entering and leaving the unit and before and after contact with the resident. The protective devices indicated by the staff are worn as instructed and are not reused outside the unit. Visits are postponed, and the staff informed, in case of fever, respiratory symptoms or gastrointestinal complaints. Food and objects brought in from outside are notified to the nursing staff beforehand. Access to rooms other than that of one's own relative is not allowed. Any further precaution indicated by the staff in relation to specific clinical situations is observed for the whole duration of the visit.

Privacy and confidentiality

The facility operates in full compliance with privacy legislation and has adopted a Privacy Manual. Confidentiality is guaranteed in the interview areas and dedicated spaces are provided for sensitive communications between physician and patient. Access to clinical information is controlled through secure IT systems and protocols are adopted to protect privacy during visits.

Rights of the resident

The facility recognises the European Charter of Patients' Rights and the fourteen rights it proclaims. In particular the following are guaranteed: access and attention, with a welcome based on courtesy, availability and attention; information on how services are provided and documentation of the services received; respect for the person, for confidentiality and for personal dignity; normality and difference, with care that does not alter the resident's habits beyond what is strictly necessary and that guarantees the specificity deriving from age, sex, nationality, culture and religion; religious assistance according to one's own faith; respect for religious dietary requirements; trust and decision, with the right of the resident or of the guardian to decide responsibly about their own life; the right to complain and to receive an answer.

Safety of the resident

Residents are supervised also in the use of restraint devices for fall prevention. Frequent mobilisation is carried out and pressure-relieving devices are used for residents at high risk of pressure ulcers, in order to preserve skin integrity and the dignity of the resident.

Foreign citizens

This quick guide is available in paper form at the reception desk, in the Italian and English editions. Staff assist foreign residents and their relatives in understanding the information provided. Requests for language support can be made at the reception desk or by writing to sanimedica@unisan.it.

Help in understanding information

At the request of the resident or of a relative, staff are available to help in understanding the information contained in the Service Charter, in this guide, in the service access guides, in the forms, in the regulations and in the rehabilitation plans. For people with cognitive or sensory disabilities alternative forms of support are provided, including direct assistance by the staff. Every form given to users includes a description that guides understanding and completion.

Meetings with a contact person

Residents and relatives may book a meeting at the offices of the facility when needed and where the organisation of the service provides for it. The quality of the service is also monitored through telephone calls, meetings and periodic meetings with the family, carried out by qualified staff by appointment.

Complaints and suggestions

Observations and complaints serve the continuous improvement of the services. A complaint may be submitted: verbally, at the premises of the facility, where it can be recorded on a dedicated form including the identification details of the person; through the dedicated procedure on the website www.sanimedica.it; by email to sanimedica@unisan.it; by ordinary letter addressed to Sanimedica, Via del Casale de Merode 8, 00147 Rome. Anonymous complaints are not taken into consideration. Observations and complaints are forwarded to the Medical Director, who carries out the appropriate checks and activates a corrective action where necessary. A written answer is given to the complainant within a maximum of 15 days.

Satisfaction questionnaires

The level of satisfaction of the resident and of the family is measured through customer satisfaction questionnaires, which residents and families are invited to complete periodically, about once a year, subject to any extraordinary surveys. The questionnaires cover the running of the facility and of the service, the staff and the relationship with the organisation.

Service quality standards

Number of complaints resolved equal to the number of complaints received. Number of complaints received not exceeding 10 per cent of the users served. Number of complaints answered in writing within 15 days equal to the number of complaints received. Number of questionnaires with a predominantly positive result equal to or greater than 90 per cent of total questionnaires, both for the relationship with the staff of the service and for the overall assessment of the services provided.

Voluntary organisations and patient protection bodies

Upon request to the health management, access may be authorised, through the signing of a protocol, for voluntary organisations entered in the regional register and operating in the area, on days, at times and in ways and spaces defined by the management. Information on the existence of and possible recourse to citizen protection bodies is ensured through the user associations present and, where these are absent, through the display on the notice board of the list of such bodies.

Where to find the documents

At the offices of the facility residents and relatives may always collect a paper copy of the Service Charter, of this quick guide and of the service access guides.
